Clinical Pharmacist - The Lennard Surgery

Job summary

The Lennard Surgery, a practice within SWIFT Primary Care Network, requires a Clinical Pharmacist to support our 8500 patient population.

Are you someone who is dynamic, energetic and positive team member that is passionate about patient care?Someone who is interested in working not only in the practice but also with patients in the community to deliver clinical excellence. We are looking for someone who is adaptable and has the ability to work as part of a GP led Multidisciplinary team.

The ideal candidate must have a flexible can do attitude with a passion for clinical excellence and an ability to work well within a team. Main duties of the job

The post-holder will look to improve patients health outcomes and the efficiency of The Lennard Surgery by providing direct, accessible and timely medicines expertise. Pro-actively transferring workload relating to medicines optimisation issues from other clinical staff to improve patient care and safety you will be freeing up other clinical staff time to enable them to meet needs of more patients with clinically complex needs.

About us

We have a dedicated and friendly team. There are four GP Partners and five Salaried GPs, two practice nurses, two healthcare assistants, a reception team consisting of two team leaders and 8 care navigators, two prescription clerks and three admin assistants. We also have a PCN Clinical Pharmacist, Paramedic, First Contact Physiotherapist and a Trainee Nurse Associate.
