Primary Care Clinical Pharmacist, full-time with occasional Saturday mornings

An exciting opportunity has arisen for a Primary Care Clinical Pharmacist under the NHS England Clinical Pharmacists in practice pilot, based in three practices in Clevedon and Portishead.

The practices are looking for a team orientated and motivated individual who is keen to develop a patient-facing role within our practices, taking responsibility for specific areas of clinical care and undertaking medication reviews to proactively manage patients with complex poly-pharmacy. The successful applicant will also provide primary support to general practice staff with regards to prescription and medication queries. They will be actively involved in prescribing audits and supporting each practice to achieve prescribing incentive scheme targets.

We need a dynamic and experienced individual who possesses excellent communication skills and who is willing to support our three practice teams with cost-effective, safe and high-quality medicines management for our population, making a positive difference to patient care and reducing workload on our general practitioners.

You will need to be well motivated, have effective time management skills and able to liaise and develop relationships at all levels with GPs, Community Pharmacists, healthcare professionals and patients.

We welcome applications from pharmacists with a minimum of 2 years post graduate experience, with a degree in pharmacy and an Independent Prescribing qualification. A comprehensive programme of training will be provided as part of the pilot scheme.
