Lead Nurse Full Time - Hillview Family Practice

Are you looking to take the next step in your career: moving to working in a small GP Practice, from community care to primary care or perhaps moving from secondary care to primary care. Whatever the reason, our practice aim is to ensure good patient care whilst considering the wellbeing of the staff a priority. Hartcliffe is known to be an area of high deprivation and whilst this brings many recognised challenges, the role also provides a great many rewards.

Our Lead Nurse is relocating which means that we are looking for a practice nurse to help make our amazing team complete again. You would lead a team of Practice Nurses and Healthcare Assistant and will undertake all aspects of the practice nurse role including cervical screening, baby immunisations and chronic disease management. Ideally you will already be an experienced nurse with management/leadership experience (this can be a supported development role) and have experience in at least one chronic disease speciality.

We offer training relevant to career progression and our lead nurse is a mentor for newly qualified nurses and for those from secondary care who wish to move into primary care.

As a core member of the leadership team your input to our practice team and our patients will be important. You will be integral to the delivery of high-quality patient care &quality improvement initiatives for our patients.

 

Main duties of the job

The Lead Practice Nurse will be responsible for the management of the practice nursing team, consisting of nurses and HCA.

They will be responsible for several clinical areas such as infection prevention and control, chronic disease management, health promotion, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

 

Overview of your organisation

We have recently undergone a successful expansion of the partnership from 2 to 4GP partners and our Clinical team now comprises an additional3 Nurses, 1 Paramedic, 1 HCA, Pharmacist, Social prescriber, Mental Health Nurse and Physiotherapist. The whole team meet daily to chat/ have lunch and support one another.
